Job Description

New Admission

  • Prompt Attendance: Attend any new patient admission within the prescribed time frame.
  • Patient History: Examine and take a proper history of the patient (if they can provide it) or from their relative.
  • Record Review: Check any previous medical records if available.
  • Documentation: Chart all patient details in the history sheet.
  • Consultant Communication: Inform the respective consultant about the admission and follow their orders.
  • Nursing Instructions: Instruct the nursing staff about necessary orders to be carried out.
  • Transfer In-Patient: Carefully read the transfer summary/clinical notes for patients transferred from the ICU and act accordingly.
  • Significant Updates: Inform respective consultants whenever a significant change or finding occurs.

Shift Duty

  • Patient Rounds: Take rounds and examine individual patients at least once in every shift and as needed. Inform the respective consultant/registrars of any new findings.
  • Clinical Notes: Document findings in the clinical sheet.
  • Patient Details: Enter all relevant details for each patient.
  • Order Implementation: Attend rounds with respective consultants, carry out necessary changes in orders, and enter them in the treatment sheet.
  • Medication Management: Fill the Medicine Card and Non-Drug Order sheet.
  • Nursing Follow-up: Instruct nursing staff about orders and follow up on their execution.
  • Investigation Tracking: Follow up on all investigations, review results, and note them in the investigation sheet.
  • Verbal Order Documentation: Document verbal orders given by consultants correctly in clinical notes.
  • Emergency Response: Respond to emergency calls immediately and activate code blue if any medical emergency is encountered beyond your control.
  • Critical Care Transfer: Identify a patient's critical status and facilitate their timely shift to the critical care area.
  • Consultant Contact: Always contact the primary consultant if deemed necessary at any point.

Discharges

  • Summary Preparation: Prepare discharge summaries well in advance according to hospital policy.

Pre-Surgical & Post-Surgical Patients

  • Pre-operative Orders: Follow pre-operative orders in the treatment sheet.
  • Consent Verification: Check whether all necessary consents (Surgical, Anesthesia, etc.) for surgery have been taken from the patient. If not, inform the respective person.
  • PAC & Orders: Get the Pre-Anesthesia Check-up (PAC) done and carry out all pre-operative orders.
  • Night Shift Reporting: Inform the consultant by phone during the night shift about the status of patients scheduled for surgery the next day.
  • Patient Support: Attend to patient complaints, calm apprehensive patients, and inform the consultant if needed.

General Responsibilities

  • Camps: Attend camps according to hospital policy.
  • Policy Adherence: Work as per the policies and procedures of the organization.
  • Cross-Referrals: Ensure cross-referral forms are filled and call cross-consultants according to policy.
  • Documentation Standards: Use capital letters for medicines (Generic Names/Salt) and always include your sign, name, date, and time with every entry you make.
  • Record Completeness: Ensure the completeness of records.
  • Management Directives: Act accordingly as communicated by the hospital management team.

Candidate Requirements

  • Work Accuracy: Able to work accurately and with minimal supervision.
  • Instruction Comprehension: Ability to comprehend both verbal instructions from supervisors/department heads and written instructions from doctors and other departmental personnel.
  • Work Organization: Ability to plan and organize one's work schedule effectively.
  • Communication Skills: Speaking and listening are essential requirements to understand and carry out instructions. Ability to give accurate and tactful explanations to patients.
  • Teamwork: Affinity to work in a team.
  • Problem-solving/Decision-making: Ability to think independently and exercise own judgment in consensus with the supervisor.
