Job Description

Key Responsibilities:

1. Transcription

  • Transcribe physician-dictated medical reports, including:
  • Patient case notes, progress notes, OPD/IPD summaries
  • Radiology/Pathology reports
  • Operative and procedural notes
  • Ensure transcription accuracy, clarity, and correct use of medical terminology.

2. Editing & Proofreading

  • Edit drafts prepared via speech recognition software (if used).
  • Proofread for grammatical, medical, and formatting errors.
  • Identify and flag any inconsistencies or unclear content for clarification.

3. Documentation & Records Management

  • Format documents according to hospital/clinic standards and templates.
  • Ensure timely and organized documentation into EMR/EHR systems.
  • Maintain confidentiality and data security of all patient information.

4. Coordination

  • Collaborate with physicians, nurses, and diagnostic teams for clarification when needed.
  • Follow up on missing dictations or incomplete records.

Qualifications & Experience:

  • Education:
  • Graduate in any discipline (preferably life sciences or English)
  • Certification/Diploma in Medical Transcription (preferred)
  • Experience:
  • 1–3 years in medical transcription or clinical documentation
  • Freshers with strong typing and medical terminology knowledge may also apply

Skills & Competencies:

  • Strong knowledge of anatomy, physiology, pharmacology, and medical terms
  • Excellent listening and comprehension skills
  • High typing speed with strong accuracy (50+ WPM preferred)
  • Familiarity with transcription/editing software and EMR systems
  • Attention to detail and confidentiality handling
  • Good command of English grammar and spelling
