Home Care Nurse

Responsibilities:

• Maintain professional nursing standards in line with current best practices.
• Ensure all patient care needs are met according to care plans, promoting safety, privacy, dignity, and confidentiality.
• Accurately record all relevant patient care and treatment information at the time care is delivered.
• Communicate with referring clinicians promptly and professionally.
• Collaborate closely with other healthcare professionals and participate in quality assurance programs.
• Provide post-discharge patient care at the patient's residence as required.
